Please arrive about 10 minutes early so that you have enough time to fill out any necessary forms. If you would prefer, you can download the forms* and fill them out in advance, and bring them with you to your first appointment.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
Initial Examination
Your dentist will begin by reviewing your medical and dental history and performing a comprehensive dental exam. Your exam will typically include a detailed examination of your gums, teeth, bite and jaw, an oral cancer screening and any required x-rays.
Treatment Plan
Based on your history and examination, your dentist will create a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.

- How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us. Please notify us of any change of plans as soon as you can, so we can offer the space in our schedule to another patient.
- Do you offer appointment reminders?
In the days prior to your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.
- Does your clinic have on-site parking?
Yes, free on-site parking is available for all our patients. We are also easily accessible by public transit.
Insurance & Financing
At Cornerstone Dental with Dr. Clark, we'll help you navigate our options for payment, direct billing to insurance companies and financing so you can focus on getting the dental care you need.
- Direct Billing to Insurance
We offer direct billing for most insurance plans. For your convenience, we are able to submit your insurance claim electronically, resulting in faster reimbursement, without requiring paper forms and signatures.
Our administrative team can assist you in understanding your insurance plan better and can also send estimates to your insurance company prior to starting any major treatments.

Any co-pays or amounts not covered by insurance will be your responsibility, and must be paid at the time of your appointment.
